Chronic Pain in the Gluteus Medius

Hello,

I have been having chronic pain in the gluteus medius for about 2 years. It is on the upper lateral hip on both sides. I have been trying everything. I believe I have pretty bad anterior pelvic tilt. I am trying to strengthen my core and glutes. Any suggestions?

If there is chronic muscle pain then, in our experience, the underlying problem is usually structural - the bones are not quite in the right place for the muscles to function properly. Based on what you see in the more traditional villages in India, Africa and other places, an anteverted pelvis is the norm wherever muscolo skeletal pain is rare. This is why we teach this in the Gokahle Method as the route to healing for most people. However there are exceptions. I cant be sure but from what you have described you may (and I stress may) have problems of alignment within the pelvis, the most common exxpression of which is SI joint pain or the kind of muscle pain you describe.  You can think of this as a problem with "settling into anteversion". If this is the case you might find that "anterior pelvic tilt" or pelvic anteversion makes the discomfort worse (these are 2 slightly different things, but now is not the time to go into that). You might find that a slight tuck of the plevis is helpful. If so then do that for now. It is only a sticking plaster solution, a palliative for  temporary use ideally, but it can be helpful. A reduction of discomfort is well worth having.

Longer term you would be best getting the "settling" issue sorted, then pelvic anteversion becomes very comfortable, but there are quite a few moving parts to this. Assuming you have yet to see a Gokhale Method teacher, the technique in the "8 Steps to a Pain Free Back" book most likely to be helpful is the Inner Corset. Another focus for your attention could be your feet, the kidney bean shaping action of the feet is very significant for settling the pelvis, but if 1 foot is stronger than the other be very careful not to exaggerate this at all - that could make things worse, so maybe Inner Corset would be the best place to start. Hope this helps.
